Acne
Acne is one of the most common skin concerns in the UK, leading to 3.5 million visits to UK primary care every year. It
can range from a few spots on the face, neck, chest and upper back, which most teenagers will experience, to a more
severe problem that can cause scarring. About 95% of people aged 11 to 30 are affected by acne to some extent and
about 3% of adults have acne over the age of 35.
Causes
The acne bacteria (now known as Cutibacterium acnes) normally lives on healthy skin, but if normal skin flora is
disrupted, bacteria can multiply and contribute to the development of acne. This can be a result of several factors:
• Excess sebum production by the sebaceous glands
• Hair follicles clogged with oil and dead skin cells
• Bacteria contaminating and infecting clogged follicles
• Hormonal changes or imbalances
• Genetic factors
• External factors such as pharmaceuticals, smoking, diet, lifestyle and stress
Ageing
From the age of 25 our body decreases its production of collagen and hyaluronic acid, and elastin fibres start to wear
out. This contributes to the loss of moisture, the dermal layer of the skin thins, wrinkles and age spots appear, skin
starts to lose its laxity and texture and there is delayed skin healing.
Causes
Factors affecting the signs of ageing include:
• Internal factors such as age, sex, genetics and hormonal balance.
• External factors such as sun exposure, pollution, pharmaceuticals, smoking, diet, lifestyle and stress.
Hyperpigmentation
Hyperpigmentation is a common medical term for darker patches of skin which can vary in size or colour. These occur
as a result of an excess production of melanin (the pigment responsible for skin’s colour) by the melanocytes (melanin
producing cells).
Sun/ Age Spots - Also known as liver spots or solar lentigines, these are caused by exposure to the sun and normally appear on older adults.
Post Inflammatory Hyperpigmentation - These are caused when skin makes extra melanin after injury or irritation such as acne, sunburn or a wound.
Melasma - More common in women, this is thought to be triggered by changes in hormone levels and pregnancy (also called pregnancy mask).
Causes
Hyperpigmentation can be a result of several factors:
• Hormonal changes
• Age
• Genetic predisposition
• Inflammation and skin injuries such as acne, sunburn or wounds
• Certain pharmaceuticals and diseases are known to cause hyperpigmentation
Please ask your patient to consult their GP if they are concerned about any dark spots especially if they notice any other
symptoms such as bleeding, itching or a change in size or colour
Rosacea
Rosacea is a fairly common inflammatory skin condition that primarily affects the central face. It’s most common in
middle-aged women and people with Fitzpatrick skin types 1 and 2. Although there is no cure for rosacea, treatment
can help control and reduce the signs and symptoms.
Causes
The exact cause of rosacea is unknown, however various theories about the disorder’s origin have evolved over the
years and include a malfunction in the innate immune system, neurovascular system disorder, vascular changes,

Factors that may trigger rosacea include:
• Temperature extremes
• Sunlight
• Hot baths and showers
• Aerobic exercise
• Stress / anxiety
• Certain food and drinks such as alcohol, caffeine, hot drinks and spicy foods
• Hormonal changes
• Certain cosmetics and pharmaceuticals
Hair loss
Human hair has a growth cycle that includes three stages: anagen (active growth phase), catagen (transitional phase)
and telogen (resting phase). About 90% of the hair on the head is in the anagen phase, which lasts anywhere between
2 to 8 years. While the intermediate and shedding stage constitutes only 10% of it.
Normal hair loss rate is fifty to one hundred strands a day. If the loss rate exceeds one hundred strands per day, it
signifies increased hair shedding and can be considered ‘hair-loss’. There are various forms of hair loss, however the
most common type is male or female pattern baldness (androgenic alopecia) which affects more than 50% of all men
aged over 50.
Causes
Hair loss can be the result of many factors:
• Genetic factors (the most common cause of hair loss)
• Age (hair cycles slow down and the effect of oxidative stress becomes more apparent)
• Hormonal changes or imbalances
• Environmental factors such as medication, medical conditions and illness (including COVID-19), poor diet, stress,
scalp health, hair styles and treatments
